Susi Q Care Management and Outreach provide important access to mental and physical health care support services for seniors. Professional care managers reach out to help older adults live safely and independently in their own homes for as long as possible. They provide in-home assessments and develop individualized care plans with the client and, if possible, family members/caretakers. They also provide referrals to home care agencies, assisted living facilities, nursing homes, transportation services, and other essential supports. What Is Care Management?
Time can bring challenges that impact our ability to live safely and independently. When this happens, we need a place to turn for guidance and support. That’s why care management (also known as case management) is a core service at The Susi Q. Our compassionate, experienced care managers assess needs and create individualized action plans. These plans include referrals to all the resources at the Susi Q and in the community. In this supportive role, our care managers guide seniors and their families and friends to solutions that reduce fear and isolation, and promote independence and community.
